If you would like us to mail an entry form to you, just let us know. We would be happy to drop one in the mail as soon as they are printed.
If you are a coach or a member of an organization that would like a batch of entry forms, just let us know how many you need and we will get them sent to you.
As soon as the entry forms are ready, you will also be able to download a pdf of the entry form and print it on your own printer.
